Enrollment

All children, regardless of race, color, religion, gender, or national origin, are welcome at The Gathering Place Polaris. To begin the enrollment process, please call us at 614.436.1395 to schedule a personal tour.

The following items are necessary for enrollment:

1. Completed registration forms
2. Child Enrollment and Health Information form
3. Child Medical Statement (to be completed by your doctor)
4. Completed EFT form
5. Paid, non-refundable registration fee
6. Paid, refundable materials fee
7. Paid, non-refundable first month's tuition
8. Other documentation as needed (ex. custody papers)

If required forms, pro-rated tuition, or additional upfront fees are not submitteed to our office either on or by your child's first day, your child will be unable to start at our center, regardless of the date listed on their registration form.
